Greenlights Referral Form

Please complete the form below to begin the referral process for access to our Greenlights behavioural support service. If you do not wish to complete the form online, you can download the form, print it, fill it in and send it by post to the address below:

Eligibility criteria

The Greenlights Project at Greenwich Mencap supports parents/carers of a child who displays challenging behaviour in the home and has one or a combination of the following diagnoses:

- A Learning Disability
- Autism Spectrum Condition (ASC)
- Attention Deficit Hyperactivity Disorder (ADHD)
- A Learning Difficulty

Additional criteria

- We support children up to age 11
- The family must be living in the Royal Borough of Greenwich (evidenced by a Greenwich postcode)

Please note that children who only display behavioural difficulties in a school setting will not be able to access support from the Greenlights Project.
